After a while, some fat cells die and get reabsorbed, but the rest get a blood supply and connect to the breast tissue. They aren't really fake at that point, it's part of the boob's fatty tissue. I don't think there is a big difference in where the fat cells came from after they are reincorporated. I think it's been long enough that this has already happened???? Is it accurate to call them fake boobs still?

It's much more popular in Asia to get the former rather than the latter, it also has a longer healing process to return to normal which is why they are finally looking better. Most plastic surgeries in Korea are done in mind to look proper after at least a year and typically 3 years is the "peak" for the work vs aging balance, not quick fixes outside of a few smaller ones like double eyelids. Jaw shaves which are the most notorious are supposed to take about 3 years for all the swelling to come down and for your body to properly redistribute fat and for your new muscular shape to compensate for the new bone structure.
